TB Clearance Check: I understand I must provide a negative TB skin test (within 12 months) or a clear chest X-ray before attending off-site clinical facility modules.

Physical Demands Agreement: I certify that I am physically capable of lifting up to 50 lbs, standing for extended periods, and executing standard clinical patient maneuvers.
